Why Sperm Health Matters for Conception
A sperm’s only job is to reach an egg and fertilise it. To do that well, the sperm needs to be in good numbers, swim in the right direction, and be shaped properly so it can penetrate the egg. If any of these pieces are off, it can take longer to conceive and, in some cases, prevent pregnancy altogether.
The good news is that sperm health is not static. It takes about 64 to 72 days for sperm cells to regenerate, so the choices a man makes today can really impact his fertility picture in a couple of months.
What Doctors Actually Measure: The Semen Analysis
Before we get to the signs, it’s useful to know how the signs are measured. The standard test of male fertility is a semen analysis. It looks at a sample of seminal fluid and assesses count, movement, and shape, following criteria set out by the World Health Organization.
This test is generally accepted as the gold standard in assessing male fertility potential and looks at several factors in the ejaculate. The WHO has revised its manual over the years, and reference ranges have been modified. The latest edition from 2021 describes the normal values as a semen volume of at least 1.4 ml, a sperm concentration of at least 16 million per ml, a total motility of 42 per cent or more, and a normal morphology of 4 per cent or more.

1. A Healthy Sperm Count
Sperm count is a measure of the number of sperm in each millilitre of semen. The NHS says a low sperm count, or oligozoospermia, is fewer than 15 million sperm in every millilitre. One of the clearest signs that your sperm is in good shape is a count above this threshold, because the more sperm there are, the better the chances some will make it to the egg.
2. Good Progressive Motility
Motility is about moving and particularly about whether the sperm swim in a straight line, in circles, or not at all. A normal semen analysis result shows that at least 50 per cent of the sperm should be moving within one hour after ejaculation. Sperms that are weak swimmers or don’t swim at all have a harder time getting through the cervix and into the fallopian tubes.
3. Normal Morphology (Shape)
The morphology is the physical structure of the sperm: head, midpiece, and tail. A normal sperm cell has a smooth oval head, an acrosomal cap that covers between 40 and 70 per cent of the head, no abnormalities in the midpiece or tail, and no cytoplasmic vacuoles that cover more than half of the head. Sperm that is anywhere near the edge of these criteria is usually counted as abnormal. This shows how strict this measure is.
4. Normal Semen Volume
Volume also counts. A normal ejaculate volume, collected after 3 to 5 days of abstinence, ranges from 1.5 to 6 mL. Too little semen (hypospermia) can mean fewer sperm delivered per ejaculation, while too much (hyperspermia) can dilute concentration.
5. Sperm Vitality
Vitality is a measurement of the percentage of live sperm in a sample. The percentage of live sperm should be more than 58 per cent. Below that, it is called necrozoospermia. And even if you have a normal sperm count and normal shape, if most of them are dead on arrival, there won't be conception.
6. Low Leukocyte Levels
Leukocytes (white blood cells) in semen may be a sign of infection or inflammation of the reproductive tract. If the leukocytes are greater than 1 million per mL, it is called leukospermia and may be a sign of a genitourinary infection. Normal counts of leukocytes in healthy sperm are usually much less than this.
7. Minimal Sperm Clumping (No Agglutination)
Agglutination – sperm clump together or stick to other cells, often a sign that the immune system is reacting against the sperm. If more than half the sperm are bunched together, it could indicate an immune-related problem. Sperm that swim freely, instead of in clumps, have a much better chance of reaching the egg without hindrance.
8. Appropriate Semen Consistency (Viscosity)
Viscosity is the thickness or thinness of the semen. If the semen is too thick, the sperm can get trapped and slowed down, but if it is in the normal range, the sperm can run free once ejaculated.
9. A Healthy Total Motile Sperm Count
Doctors do not only look at motility percentages. They also look at the total motile sperm count, or TMSC, which combines the concentration and motility into one number. In couples undergoing intrauterine insemination, the success rate is significantly higher when the TMSC is between 5 and 10 million than when it is less than 5 million (see Table 10.2). Together, this gives a better picture of fertility than looking at either count or motility alone.
10. Balanced Mineral Composition
Semen chemistry also plays a lesser role. Studies have shown that even men with normal semen analysis may have early hyperactivation of sperm in association with deficiencies of elements such as potassium, calcium, copper and zinc. It’s a more complex marker, but it does suggest that healthy sperm is more than just count and shape. The chemistry inside is important too.
11. Low Sperm DNA Fragmentation
Sperm DNA fragmentation tests check for damage to the genetic material that is carried inside a sperm. Genetic instability is not detected by simple semen analysis, and some subfertile men are misclassified as fully fertile when a semen analysis appears normal. Generally, reduced DNA fragmentation is associated with better embryo development after fertilisation.
12. A Reasonable Abstinence Window Before Testing
This is less about the sperm and more about getting an accurate reading of it. In a large study of > 4,000 men, longer abstinence was associated with higher sperm concentration overall but also reduced progressive motility in both normal and abnormal samples. Most clinics recommend that men abstain for two to five days before providing a semen sample. This range of time will give a good balance between count and motility.
13. No Signs of Oxidative Stress
Sperm cells are vulnerable to damage by free radicals or reactive oxygen species that originate within. High oxidative stress is one of the causes of poor sperm quality, especially in men with lifestyle factors such as smoking and heavy drinking. Generally, the lower the oxidative stress markers in a semen sample, the better the sperm structure and motility.
